Complete the following form if you would like Transitions, Inc. to release your records to you or an individual or group (for example, a doctor, lawyer, insurance company, etc.)
You can expect to hear from us within one week of your request; however, if we are currently receiving a high volume of requests, it may take up to 30 days.
Transitions, Inc. must have a valid Release of Information signed by the client in order to complete this request.
A Release of Information from your organization or the Transitions ROI form provided below can be faxed to 859.356.0616 or emailed to records@transitionsky.org.
