*In September 2016, Transitions Inc. Administrative Office and Outpatient Services combined and moved into our new location in Covington, allowing the agency to increase capacity and serve more individuals in our outpatient programs.

*In October 2016, Transitions Inc. received 3-year CARF National Accreditation, demonstrating Transitions’ commitment to continuously improve service quality and to focus on the satisfaction of the persons served.

*In November 2016, Transitions, Inc. responded to the needs of the state and implemented Targeted Case Management and Mental Health Therapy for pregnant and parenting women with Opiate Use Disorders.

*In January 2017, Transitions, Inc. increased our Non-Medical Detox Program from thirteen to fifteen beds to meet the community’s need for increased detox services.

*Currently, Transitions Inc. is in the final implementation stages with St. Elizabeth’s to provide Substance Use Counseling services in the Covington Emergency Department to ensure a safe handoff to treatment after a patient has overdosed.

*CEO and Leadership Team at Transitions continue to advocate at the state-level to implement the 1115 Waiver and remove the 16 bed IMD Exclusion in the state of Kentucky.

Transitions, Inc. was founded in 1969 by volunteers concerned about the lack of substance use disorder treatment for the uninsured in Northern Kentucky. Today, we offer women and men a holistic continuum of care comprised of non-medical detoxification, residential treatment, outpatient treatment, case managed sober housing and aftercare services. By collaborating and partnering with other local agencies, Transitions strives to be the lead Treatment Providers in the Northern Kentucky community.

Transitions, Inc. is seeking to find certified and licensed counselors responsible for assessment, diagnosis, treatment planning and evaluation and counseling services at our various locations.

Position Title:            Advanced Counselor


Supervisor:                Treatment Supervisor


Qualifications:           Bachelor’s degree; and Active Kentucky License as a Certified Drug and Alcohol Counselor (CADC) and/or

                                    Master’s degree; and Active Kentucky License as a Certified Social Worker (CSW) or Licensed Professional Clinical Associate (LPCA)



Position Summary:


The Advanced Counselor is responsible for maintaining a caseload of clients and may provide assessment, diagnosis, treatment planning and individual and/or group clinical services. Services may be provided in a variety of settings. A dependent license is required.



Job Responsibilities:

*Interview and observe clients and prepares clinical reports and individualized service plans

*Provide cognitive behavioral treatment, conduct assessments (including dependent diagnosis), individual, group counseling, and case management.

*Assure documentation is in compliance with contract regulatory and agency requirements and best clinical practices.

*Use approved counseling methods in order to maximize treatment process and outcomes (group and individual).

*Use approved interviewing techniques and/or instruments in order to develop an accurate diagnostic summary/service plan.

*Complete clinical documentation within expected timeframes.

*Stay current with updated research and best practices.

*Participate in individual and/or group supervision and staff trainings as required.

*Represent Transitions, Inc. in a positive and professional manner in all community interactions.

*Assists in other Transitions’ programs as needed or assigned.
